Use the tool to strengthen reflection, skill-building, and self-directed action between appointments, while maintaining appropriate clinical boundaries and oversight. The tools are not a substitute for assessment, diagnosis, treatment planning, or professional judgement. Practitioners should apply it within their professional scope of practice, adapt it where necessary, and consider whether additional support, referral, or risk management is required. These resources are designed to support appropriate patients as part of a broader, clinically informed care plan. It should be used only where it is relevant to the person’s needs, presentation, capacity, and current level of safety.
